Abstract	Recent developments in the telecommunications and the information technology fields hold the promise of improved access to and better utilization of an integrated health care related resources. In addition, to these developments, the deployment of interactive distant training programs offer an opportunity to decrease the knowledge gap between the leading academic medical centers where new medical expertise is continuously being discovered and remote health care practitioners who find themselves pressed to deliver quality care that meets the needs of their communities in a competitive environment of limited resources. The implementation of Telehealth programs in such an environment is supported by enthusiastic approaches, but limited systematic planning. There is a need to be able to adopt the advantages offered through intelligent communications that extends our senses, records our activities, and limitlessly stretches our outreach. At its basic application, Telemedicine can take the distance out of caring. Clinical needs, including provision for innovative quality continuing education programs, are one of the plan's prime focuses. However, once we move beyond the clinical, legal and financial implications of telemedicine, we find that the sustainable practice is still needed to overcome the engineering challenge. If telemedicine is going to become a staple of medical practice in our planet, a massive re-engineering of the national and international healthcare telecommunications infrastructures, of financing methodology and of clinical protocols will be required.
