| Abstract | OZET Amac: Bu cal›flman›n amac› okluzal curuk teflhisinde gozle muayene, geleneksel bite-wing radyografi ve direkt dijital radyorafi et- kinliklerinin referans deer olarak kabul edilen histolojik deerlendirme kriterleri ile karfl›laflt›r›larak araflt›r›lmas›d›r. Gerec ve Yontem: Cal›flmada 250 adet periodontal veya protetik amacla cekilmifl, gozle gorulur kavitasyonu olmayan insan daimi buyuk ve kucuk az› difli kullan›ld›. Difllerin gozle muayeneleri ve standart flartlarda cekilen geleneksel ve dijital radyograflar›n›n deerlendirmesi iki gozlemci taraf›ndan birbirinden ba¤›ms›z olarak yap›ld›. Daha sonra difllerden al›nan kesitler steromikroskop alt›nda X10 buyutmede incelenerek curuk seviyeleri histolojik olarak belirlendi. Gozle muayene, geleneksel radyografi ve dijital rad- yografi skorlar›n›n birbiri ile ve referans deerimiz olan histolojik deerlendirme sonuclar› ile uyumu ve iki gozlemcinin sonuclar›- n›n birbiriyle uyumluluu istatistiksel olarak Kappa analizi ile deerlendirildi. Yontemlerin spesivite ve sensitivite deerleri hesap- land›. Bulgular: Her iki gozlemcinin Kappa deerleri incelendiinde referans deer (histolojik deerlendirme) ile en iyi uyum gosteren yon- temin gozle muayene olduu saptanm›flt›r. Gozle muayene, geleneksel radyografi ve dijital radyografi icin spesifite deerleri s›ras›y- la; 1. gozlemci icin 0.83, 0.82 ve 0.74 iken 2.gozlemci icin 0.46, 0.79 ve 0.61, sensitivite deerleri ise 1.gozlemci icin 0.44, 0.29 ve 0.34 ikinci gozleci icin 0.85, 0.29 ve 0.51'dir. Sonuc: Cal›flman›n sonuclar›na gore, ozellikle bafllang›c okluzal curuklerinin teflhisinde test edilen tekniklerden hic birinin tek bafl›- na guvenilir olmad›¤›n› yontemlerin bir arada kullan›lmas›n›n doru teflhis oran›n› artt›rabileceini soyleyebiliriz. Anahtar Kelimeler: Okluzal curuk teflhisi, gozle muayene, geleneksel radyografi, dijital radyografi, histolojik deerlendirme SUMMARY Objective: The aim of this study was to evaluate the diagnostic efficiencies of visual inspection, conventional and direct digital ra- diography for occlusal caries detection in comparison to the histological validation as gold standart. Material and Method: The study comprised extracted 250 human premolar and molar teeth with visually intact occlusal surfaces . Visual inspection and evaluation of conventional and digital radiographs that are taken under standardized conditions were carried out by two observers independently. Than the teeth were sectioned and the sections were examined by steromicroscope under x10 mag- nification for histologic validation. The concordence between visual inspection and radiographic and histological validations and in- terobserver reliability were evaluated with Kappa statistics. Sensitivity and specificity values were calculated for all methods tested in the study. Results: According to the Kappa values, visual inspection had the best concordence with the gold standart (histologic validation) for both two observers. The respective specivity values for visual inspection, conventional radiography, and digital radiography were 0.83, 0.82, 0.74 for the first observer and, 0.46, 0.79 and 0.61 for the second observer and sensitivity values were 0.44, 0.29 and 0.34 for the first observer and 0.85, 0.29 and 0.51 for the second observer. Conclusion: According to the results of the study we concluded that none of the methods tested gave accurate results for the detec- tion of especially early occlusal caries when used seperately, combination of the methods may increase the rate of true diagnosis. |
