| Abstract | The team of the Basic Family Health Unit (UBS) in the São José neighborhood of Ipaba (MG) identified as the main challenge the inappropriate management of spontaneous demand, especially for individuals with arterial hypertension. He also noticed the harmful reflex of this management in the management of the demand for chronic non communicable diseases (DCNT), and decided to intervene resolutely, beginning with investigating and classifying the cardiovascular risk for acute coronary event in the population enrolled, in the age group of 30 Up to 74 years, using the Framingham score. He proposed the application for both men and women. Identified the population at risk, this team intends to stratify it into levels of necessity and priority, in order to propose the implementation of the flowchart for the tracing and diagnosis of arterial hypertension in order to guide the work process of the Health team of the family. In order to promote a certain innovation in the process, the team agreed with the need to monitor not only the application of the process itself, but also to record the longitudinal results measured, taking advantage to create a tool that would allow a broad vision that favored the elaboration More appropriate to the situation of each client. Therefore, a spreadsheet for individual monitoring was elaborated, allowing a longitudinal and strategic visualization, allowing, therefore, the construction of appropriate and personalized interventions, according to the need of each accompanied user, in order to increase the effectiveness of the attention and Care. |
