| Abstract | Pacientes com doenca renal progressiva e diabetes mellitus apresentam geralmente hipertensao arterial grave e de dificil controle que piora ainda mais seu prognostico renal e cardiovascular. As possiveis causas da dificuldade do controle da pressao arterial sao hipervolemia, aumento da rigidez arterial, disfuncao endotelial, estenose de arteria renal associada e regime terapeutico inadequado. O reconhecimento dessas condicoes e a abordagem medicamentosa mais adequada podem determinar o alcance das metas pressoricas relacionadas a menor risco para os pacientes. Patients with progressive renal disease and diabetes frequently exhibit severe and difficult-to-control arterial hypertension that worse even more their cardiovascular and renal prognosis. Possible causes for the un controlled blood pressure include volume excess, increased arterial stiffness, endothelial dysfunction, associated renal artery stenosis and inadequate therapeutic regimen. The acknowledgement of these conditions and the adoptionof a more effective antihypertensive treatment can help to reach pressures goals related to a lower risk for thepatient. |
