| Abstract | This paper discusses how the relationship between state and social actors participate in defining public policy. The case analyzed is the Incentive Policy of the National STD / AIDS, the Brazilian Ministry of Health. Health policy in question is part of a set of actions promoted by the Ministry of Health to control the AIDS epidemic in the country. The Brazilian system of epidemiological surveillance, the HIV / AIDS area, has been recognized worldwide for its efficiency, expanding access to health services to the population, and especially the realization of preventive actions in partnership with civil society organizations. The Incentive Policy became effective in 2003 changing the organization and implementation of health activities in the area. The main change due to policy refers to the displacement of management functions from the federal to the state and municipal levels. NGOs / AIDS that related directly to the federal health managers also began to interact with the city managers in shaping local actions. This study examines, from a case study, the effect / result of this shift and its implications for the construction of politics at the municipal level. In the study, it was found that NGOs / AIDS lost power to influence the definition of the shares on the disease and the municipal health officer grew in importance, the extent to which such actions were incorporated into the National Health System instead of holding through a parallel system as established previously. The decentralization generated by the Incentive Policy led the municipal manager to exercise command over the definition of actions in the area and reduced the participation of NGOs / AIDS in the process of forming the same.
