"How do you do it?" is something that, I suspect, all of us in the healthcare professions have heard at one time or another. How many of our friends and family simply cannot fathom the horror of traumas that we see daily as part of our "normal" routine, our familiarity with diseases like cancer and mental illness that they find impossible to imagine? But this time, the questioner was not an uninitiated lay person; she is a skilled nurse with many years of emergencydepartment experience. Her comment-referring to the fact that my caseload consists heavily of people with AIDS-came from knowing that I had lost, within three weeks of each other, my life partner and my best friend to AIDS, and that, as a gay man, I could hardly count the number of friends and acquaintances this monster illness had taken away. I was surprised to find my response taking me back to college nearly 20 years ago and to something Jean-Paul Sartre wrote on the subject of the Nazi occupation of France. Sartre argued that the French people "were never more free than under the German Occupation." So completely devastated were their lives, he believed, that their every action-indeed, their every thoughtwas raised to the level of a battle between freedom and acquiescence, between life and death.
