| Abstract | RESUMEN Objetivo : Determinar los niveles de adiponectina en pacientes con sindrome metabolico y su correlacion con el fenomeno de insulino resistencia. Metodos : Estudio experimental, de tipo transversal, comparativo casos – controles, que determino la prevalencia de sindrome metabolico en una muestra de 70 pacientes masculinos escogidos al azar, que acudieron a la consulta externa de medicina vial en Ciudad Bolivar con edades entre 25 – 65 anos; tambien se determinaron los niveles de adiponectina en plasma de dicha poblacion y se evaluo en ellos la resistencia a la insulina mediante el metodo Homeostasis Model Assessment. Resultados : De los 70 pacientes mas- culinos elegidos como muestra, 30 de ellos (42,85 %), cumplian con al menos 3 de los 5 criterios de inclusion de la ATP III para ser catalogados con sindrome metabolico. Se determino la concentracion plasmatica de adiponectina y se encontro que era de 10,45 @ 2,14 ng/mL, a diferencia de los pacientes control los cuales presentaron una concentracion plasmatica de 70 @ 3,20 ng/mL observandose diferencias significativas entre ambos grupos (P< 0,01). Adicionalmente estos pacientes exhibieron resistencia a la insulina cuantificada por el metodo Homeostasis Model Assessment a tal punto que los pacientes del grupo control presentaron un indice Homeostasis Model Assessment de 1,20 y los pacientes con sindrome metabolico un Homeostasis Model Assess- ment de 3,22. Conclusiones : Se encontro una alta prevalencia de Sindrome Metabolico en la poblacion estudiada, asi como una baja concentracion plasmatica de adiponectina lo cual se correlaciono con un alto indice Homeostasis Model Assessment IR, quedando demostrado de esta manera que en los pacientes con este sindrome se presentaba el fenomeno de insulino resistencia el cual interfiere en el metabolismo de los carbohidratos y grasas. ABSTRACT Objectives : To measure the plasma adiponectin levels in patients with metabolic syndrome and its relationship to insulin resistance. Methods : Transversal, comparative case - control study. We conducted a survey of 70 male patients chosen at random to determine the prevalence of metabolic syndrome; they were selected from an ambulatory setting in Ciudad Bolivar, Venezuela, ages 25 to 65 years- old. We used the enzyme linked inmunoabsorbent assay method for the determination of plasma adiponectin levels and we also determined insulin resistance with the Homeostasis Model Assessment method. Results : From a sample of 70 male patients 30 (42.85 %) had 3 of the major components of the metabolic syndrome according to The National Cholesterol Education Program Adult Treatment Panel III (NCEP-ATPIII). The plasma adiponectin levels in patients with metabolic syndrome were 10.45 @ 2.14 ng/mL and 70 @ 3.20 ng/mL in the control group with a significant difference between both groups and statistical significance (P< 0.01). Additionally these patients exhibited insulin resistance determined using the Homeostasis Model Assessment method: in the control group the index was 1,20 and in the patients with metabolic syndrome the Homeostasis Model Assessment index was 3.22. Conclusions : We found a high prevalence of metabolic syndrome in the studied popula tion, decreased plasma adiponectin levels which correlated with high index of Homeostasis Model Assessment IR. The insulin resistance believed was strongly associated with atherogenic dyslipidemia and interference with carbohydrate and fat metabolism. |
