Abstract	This article presents experiences and reflections about the importance of the Home Visit as a modality of health care integrated into Health Care Networks (RAS), characterized by a set of actions to prevent and treat diseases, rehabilitation, palliation and health promotion provided at home ensuring continuity of care. From a situational diagnosis, it was observed that there was in the team work process a plan for home visits programmed for bedridden patients, puerperal, in addition to people at risk. In view of this situation, it was observed the need to elaborate an intervention Plan with the objective of guaranteeing the continuity of care to this group of patients. In order to carry out this intervention, it was necessary to use a Situational Strategic Planning as follows: initially identifying the problems of people in need, selecting the priority, describing and discussing the operations that should be performed and the respective strategies for obtaining of successful implementation of the action plan aimed at improving the implementation of home visits, assisted by the health team of the Basic Health Unit UBS.
