| Abstract | ABSTRACT The impact of the intervention of the Pharmaceutical Chemist on the pharmacotherapeutic follow-up was determined by means of 4 parameters, in patients with a diagnosis of arterial hypertension, who attended the Union Health Center. Eight home visits were performed during a period of 9 months, using an adaptation of the DADER method, specific for this study, with the aim of improving the health outcomes. Thirty-six patients aged 42 years and older with a diagnosis of essential hypertension were included in the study. A 100% adherence was achieved (p <.01), with the second Morinski-Green-Levine test, a Z test of proportions with a total of 8.49 and p <.01 was performed for each question. The Purdue Pharmacist Directive Guidance Scale showed an improvement of 85.7% to 100%, verified with the Wilcoxon test with results in positive ranges, with a p <.01. In the blood pressure monitoring, the results were obtained using the Friedman test. The t-student test was used to determine the means and standard deviations of the aforementioned parameters. A steady decline in systolic (138.6 to 118.8 mmHg) and diastolic (87.1 to 79.3 mmHg) levels were observed. |
