Abstract	La sécurité des patients demeure l’une des principales questions abordées par les politiques de santé et le débat public. De forts taux d’erreurs dans l’administration des soins médicaux ont été révélés à plusieurs reprises, notamment dans le rapport phare de l’Institut de médecine qui estimait que les décès dus à une erreur médicale étaient plus nombreux que ceux imputables à un accident de la circulation ou un cancer du sein (Kohn et al., 2000). Une comparaison robuste des performances entre pairs est essentielle pour créer les conditions d’une amélioration. Deux types d’incidents en lien avec la sécurité des patients peuvent être distingués : les événements qui ne devraient jamais se produire comme l’oubli d’un corps étranger (par exemple aiguilles, lames, gaze) à la fin d’une intervention chirurgicale ; et les incidents préjudiciables, comme la septicémie postopératoire, qui ne peuvent pas être évités dans tous les cas, compte tenu du caractère risqué de certaines interventions, même si une hausse des taux de complications peut être le signe de problèmes systémiques.
