Abstract Malgré certains progrès, les États-Unis ne sont apparemment pas très bien placés parmi les pays de l’OCDE pour ce qui est de diverses mesures des résultats de la santé. Or, les dépenses de santé y sont sensiblement plus élevées que dans tout autre pays de l’OCDE. Tandis que des facteurs extérieurs au système de santé contribuent à ce déficit de performance, il existe aussi probablement des possibilités d’améliorer la santé des Américains tout en réduisant, ou du moins en n’augmentant pas, les dépenses. Le présent chapitre examine plus particulièrement deux éléments qui contribuent à la divergence entre les résultats et les dépenses en matière de santé aux États-Unis : l’accès inéquitable aux services médicaux et aux polices d’assurance privées subventionnées, et le manque d’efficacité de l’assurance-maladie publique. Il propose ensuite deux séries de réformes propres à améliorer le système de santé des États-Unis. La première est un ensemble de mesures destinées à assurer une couverture maladie quasi universelle. La deuxième concerne les méthodes de paiement et les décisions de prise en charge au sein du programme Medicare et vise à réaligner les incitations et à renforcer l’évaluation économique des différents actes médicaux.

National Digital Library of India (NDLI) is a virtual repository of learning resources which is not just a repository with search/browse facilities but provides a host of services for the learner community. It is sponsored and mentored by Ministry of Education, Government of India, through its National Mission on Education through Information and Communication Technology (NMEICT). NDLI is designed to hold content of any language and provides interface support for 10 most widely used Indian languages. It is built to provide support for all academic levels including researchers and life-long learners, all disciplines, all popular forms of access devices and differently-abled learners. It is designed to enable people to learn and prepare from best practices from all over the world and to facilitate researchers to perform inter-linked exploration from multiple sources. It is developed, operated and maintained from Indian Institute of Technology Kharagpur.
