Abstract In July 1981, two of three skywalks in the atrium of the Hyatt Hotel located in Kansas City, Mo., collapsed, resulting in 113 deaths and 186 injuries. This catastrophic loss has been the subject of numerous investigative reports, news specials, and engineering reviews. This paper briefly reviews the facts and circumstances leading up to the failure so that readers will better appreciate the problems that arose from the complex interactions between design, specifications, construction, and quality control. Finally, the question of “Do we really learn from our mistakes?” is explored.
