Abstract	Rev Med Liège 2015; 70 : 1 : 17-21 Résumé : Cet article résume la place de l’imagerie et des tests de laboratoire dans le diagnostic de l’embolie pulmonaire selon les recommandations récentes de la Société Européenne de Cardiologie. En cas d’instabilité hémodynamique, la démonstration d’une dysfonction/surcharge ventriculaire droite sévère à l’échocardiographie justifie un traitement de reperfusion en urgence. En cas de stabilité hémodynamique et de faible probabilité d’embolie pulmonaire, une augmentation significative des D-dimères justifie, en seconde intention, la réalisation d’un angioscanner thoracique. En cas de haute probabilité clinique d’embolie pulmonaire, l’angioscanner thoracique spiralé devient l’examen de première intention. La scintigraphie de ventilation-perfusion est une option valable pour les patients ayant une contre-indication à l’angioscanner thoracique. mots-clés : Embolie pulmonaire Imagerie Recommandations Diagnosis of acute pulmonaRy embolism. euRopean guiDelines summaRy : This article summarizes the role of imaging techniques for the diagnosis of pulmonary embolism, according to the European Society of Cardiology 2014 guidelines. In case of hemodynamic instability, bedside echocardiography is the most useful initial test. In case of hemodynamic stability, plasma D-dimers measurement represents the first step and computed tomographic angiography is the second line test, except if pulmonary embolism is likely; then, computed tomographic angiography is the first choice test. Otherwise, lung scintigraphy remains a valid option for patients with elevated D-dimers and a contraindication to computed tomography. KeywoRDs : Pulmonary embolism Imaging Guidelines A.
