Abstract	The basic attention is a health care model to meet the health needs of the people, from participatory practices and teamwork. This study emerges from an analysis of the family health strategy José Guilherme Campos, located in rural municipality of Maria da Fé, Minas Gerais, on the main problems and difficulties for the team for their minimization. This analysis resulted in various problems, however, the ineffective control of the treatment of Diabetes Mellitus among people of the areas concerned is the priority. The control ineffective corroborates to raise the rates of complications and is therefore multicausal relates to the degree of education, ignorance about the disease and treatment; the difficulty of incorporating the chronicity in living; the Organization of the work process that hinders the longitudinal follow-up, access to health services and addressing the needs of people with Diabetes Mellitus. From a collective work and based on current Brazilian guidelines of Diabetes Mellitus and the principles of operating group of Pichon Riviere was drafted a contingency plan built from the project entitled: Alive and Well two subprojects: United by health, which aims to improve the adhesion of the people to pharmacological treatment and pharmacological, and Communication, aimed at the longitudinal follow-up of people with Diabetes Mellitus and the access to nursing and medical consultations. This action plan constitutes a viable and feasible alternative that contributes to improve the adhesion of the people to treatment, minimizing its complications and to improve the quality of life.
