| Abstract | It was found change in the profile of the population mortality with an increase in deaths by chronic diseases, having great impact the complications of diabetes and hypertension. It is known that most of these cases can be treated in primary health care services through preventive measures and health promotion. This study aims to identify the reasons related to the harms of these diseases in Equipe de Saúde da Família of Silvestre, in Viçosa, Minas Gerais. The methodology was based on literature review, prioritizing texts that elucidate the problem and critical points: knowledge, habits and lifestyle, working process of the health team and irregular use of medications. As a result, an Intervention plan with goals and actions was created. The main actions were creation of both Operational Group of Hypertension and Diabetes and HIPERDIA, in the medical agenda. The expected results include cutback in number of medical appointments due to harms caused by HBP and diabetes, improve patient’s perception about his own disease, promote self-care and maintenance of HIPERDIA in the medical agenda. It is concluded that the identification of major health problems in the community through the situational diagnosis and the definition of efficient and effective interventions developed by the whole health team, are essential to the planning of actions. Through this technology problems can be faced in a systematic way, which is less improvised and has gotten more chances to succeed. Key-words: Hypertension. Diabetes Mellitus.
