| Abstract | ABSTRACT Introduction. The scarcity of Gabonese data on hepatic abscess justified this work. Its objective was to establish the epidemiological and progressive profile of hepatic abscess. Patients and methods. This is a retrospective cross-sectional study carried out at the hepato-gastroenterology department of the CHU of Libreville between January 1, 2018 and March 31, 2019. We included hospital patients with liver abscess imaging. We have collected socio-demographic, diagnostic, therapeutic and evolutionary data from patients. The Epi-info software version 7.1.5.0 allowed the statistical analysis. Results. We have collected 16 cases in 15 months. The sex ratio was 1.3. The average age was 47 years old. The unemployed represented 68.8%. A field of chronic alcoholism (62.5%), diabetes (12.5%) and HIV (6.3%) of patients. The patient presented with pain in the right hypochondrium (93.8%), fever (68.8%) and hepatomegaly (68.8%). The abscess was unique in 87.5% of cases and sat on the right lobe. Its size was between 5cm and 10cm in 68.8% of the cases and more than 10cm in 31.2%. Patients presented with leukocytosis (87.5%), anemia (62.5%), positive CRP (75%), significant amoebic serology (75%), positive blood culture (43.8%), examination cytobacteriological pus positive (50%). 100% of abscesses were drained by percutaneous ultrasound-guided aspiration. There were 6.3% of deaths 12.5% of need for additional surgical drainage. Conclusion. The liver abscess affected a 47-year-old unemployed man who was willing to drink. The painful hepatomegaly associated with fever was its clinical presentation. The amoebic etiology was predominant and the prognosis was reserved. RESUME Introduction. La rarete de donnees gabonaises sur les abces hepatique a justifie ce travail. Il avait pour objectif d’etablir le profil epidemiologique et evolutif de l’abces hepatique. Patients et methodes. Il s’agit d’une etude transversale retrospective, realisee au service d’hepato-gastroenterologie du CHU de Libreville entre le 1e janvier 2018 et le 31 mars 2019. Nous avons inclus les patients hospitalises presentant une imagerie d’abces hepatique. Nous avons collecte les donnees sociodemographiques, diagnostiques, therapeutiques et evolutives des patients. Resultats. Nous avons collige 16 cas en 15 mois. Le sex-ratio etait de 1,3. L’âge moyen etait de 47 ans. Les chomeurs representaient 68,8%. Un terrain d’alcoolisme chronique (62,5%), de diabete (12,5%) et de VIH (6,3%) des patients. Le patient presentait une douleur de l’hypochondre droit (93,8%), une fievre (68,8%) et une hepatomegalie (68,8%). L’abces etait unique chez 87,5% des cas et siegeait sur le lobe droit. Sa taille etait comprise entre 5cm et 10cm dans 68,8% des cas et plus de 10cm dans 31,2%. Les patients presentaient une hyperleucocytose (87,5%), une anemie (62,5%), une CRP positive (75%), une serologie amibienne significative (75%), une hemoculture positive (43,8%), un examen cytobacteriologique du pus positif (50%). 100% des abces ont ete draines par aspiration percutanee echoguidee. Il y a eu 6,3% des deces 12,5% de necessite de drainage chirurgical complementaire. Conclusion. L’abces hepatique touchait l’homme de 47 ans, chomeur, volontier alcoolique. L’hepatomegalie douloureuse et la fievre etaient sa presentation clinique. L’etiologie amibienne etait predominante et le pronostic etait reserve. |
