| Abstract | 主な結果 Mantel-Haenszel法による固定効果モデルにより,pooledオッズ比 (odds ratio: OR) を算出。試験間の異質性が大き いときにはランダム効果モデルも使用。 11試験 (27115例: abciximab群 12602例,対照群 14513例) が対象となった。うちprimary経皮的冠動脈インターベン ション (percutaneous coronary intervention: PCI) 実施は8試験 (3949例) で,線溶療法実施は3試験 (23166例) で あった。 30日後の死亡率 (主要エンドポイント) については,両群間で有意差はみられなかった (abciximab群 5.2%,対照群 5.5%,OR 0.97,95%信頼区間 [confidence interval: CI] 0.87~1.08,P=0.61; P for heterogeneity=0.38)。線溶療法 を行った試験の解析においても両群間で有意差はみられなかったが (5.8%,5.8%,OR 1.0,95%CI 0.9~1.12,P= 0.95,P for heterogeneity=0.25),primary PCIを行った試験の解析では,abciximab群で死亡率は低下した (2.4%, 3.4%,OR 0.68,95%CI 0.47~0.99,P=0.047,治療必要人数 [number needed to treat: NNT] 100,P for heterogeneity=0.77)。 長期死亡率 (6~12ヵ月後) についても,両群間に有意差はみられず (7.9%,8.0%,OR 0.88,95%CI 0.71~1.09,P= 0.25,ランダム効果モデルより; P for heterogeneity=0.03),線溶療法を行った試験においても同様であったが (8.6%,8.3%,OR 1.04,95%CI 0.95~1.15,P=0.41; P for heterogeneity=0.15),primary PCIを行った試験では, abciximab群で死亡率は低下した (4.4%,6.2%,OR 0.69,95%CI 0.52~0.92,P=0.01,NNT 55.6; P for heterogeneity Copyright(c) All rights reserved by Minds, Japan Council for Quality Health Care |
