| Abstract | Doenca de Crohn (DC) e uma doenca inflamatoria intestinal cronica de causa desconhecida que afeta qualquer porcao do trato gastrintestinal, da boca ate o ânus. Assim, o objetivo deste trabalho foi relatar um caso clinico da DC complicada com fistulas de um paciente, alem de descrever o tratamento farmacologico, monitorar os parâmetros bioquimicos e fatores que possam ter contribuido para a remissao da doenca. Este trabalho consiste em um estudo descritivo. Os dados foram coletados atraves do prontuario e prescricao medica nos meses de janeiro a junho de 2015. Paciente F.A.P.N., 59 anos, 47,4 kg, presenca de fezes na urina ha quatro anos e fistula colo-vesical. Foi internado com quadro de desidratacao, alem de estar desnutrido, hipocorado, astenico, anicterico e edemaciado. Apresentou abdome distendido, flacido e indolor a palpacao e multiplos orificios com fistulas enterocutâneas, no qual foi submetido a alguns procedimentos cirurgicos. Realizou-se o acompanhamento farmacoterapeutico e observou-se que o paciente manteve quadro de anemia, hipoalbuminemia e apresentou agudizacao do quadro inflamatorio demonstrado pelos sinais clinicos e pelo resultado da proteina C reativa elevada. O paciente fez uso de antibioticoterapia, imunossupressor e adalimumabe para controle e manutencao da remissao da DC. No entanto, apos recorrentes infeccoes hospitalares, teve piora do quadro clinico e foi a obito. Apesar do desfecho negativo, e possivel inferir que o diagnostico preciso, tratamento precoce e acompanhamento farmacoterapeutico, constituem fatores determinantes para remissao da DC e melhoria da qualidade de vida. PHARMACY CLINIC AND COMPLICATED CROHN'S DISEASE: CASE ABSTRACT Crohn's disease (CD) is a chronic inflammatory bowel disease of unknown cause that affects any portion of the gastrointestinal tract, from the mouth to the anus. Thus, the objective of this study was to report a clinical case of CD complicated with fistulas of a patient, besides describing the pharmacological treatment, to monitor the biochemical parameters and factors that may have contributed to the remission of the disease. This work consists of a descriptive study. The data were collected through medical records and prescription in the months of January to June of 2015. Patient F.A.P.N., 59 years old, 47.4 kg, presence of faeces in the urine for four years and colo-vesical fistula. He was hospitalized with dehydration, besides being malnourished, hypochromized, asthenic, anicteric and swollen. She had a distended abdomen, flaccid and painless palpation and multiple orifices with enterocutaneous fistulas, in which she underwent surgical procedures. Pharmacotherapeutic follow-up was carried out and it was observed that the patient maintained anemia, hypoalbuminemia and presented an exacerbation of the inflammatory picture, demonstrated by clinical symptoms and elevated C-reactive protein. The patient made use of antibiotic therapy, immunosuppressant and adalimumab to control and maintain the remission of Crohn's disease. However, after recurrent hospital infections, the clinical picture worsened and he died. Despite the negative outcome, it is possible to infer that accurate diagnosis, early treatment and pharmacotherapeutic follow-up are decisive factors for remission of CD and improvement of quality of life. |
