| Abstract | Background: A number of surgical approaches have been devised for the treatment of atrial fibrillation (AF). The ”classical” Maze procedure, although successful in the eradication of AF in high percentage of cases, was invasive and associated with significant morbidity. Recent investigators suggest, that in many patients AF may be caused by reentry wavelets limited to specific areas near the origins of the pulmonary veins. The arising technologies are moving towards transmural ablations, less invasive procedures and are involving also the ”non mitral” patients whose proportion is constantly growing. Material and methods: The ablation procedure was performed with Cardioblate ® BP Medtronic ablation system in 6 patients as a concominant procedure to of pump coronary artery bypass, aortic valve replacement with coronary artery bypass grafting and aortic valve and ascending aorta replacement. Results: During the hospitalisation and at the day of discharge one patient had atrial fibrillation and all the other patients were in sinus rhythm. There were no complications related to the ablation. Conclusions: The method of epicardial pulmonary vein isolation with use of Cardioblate ® BP Medtronic ablation system is easy, repeatable and safe. The method extends the group of patients in whom the atrial fibrillation can be surgically treated, since it can be a concominant procedure to any operation including off-pump surgery. The mid- and long-term follow-up of these patients is warranted. (Folia Cardiol. 2004; 11: 373–377) |
