Evaluación de la utilidad clínica de la prueba ELISA de detección de Ag manano de Candida para el diagnóstico de la candidiasis invasiva en pacientes pediátricos

| Abstract | Context: Invasive candidiasis (IC) is the fourth leading cause of nosocomial bloodstream infections and associated mortality is greater than 40%. Diagnosis based on an immunoenzymatic technique, allows detection at an appropriate time, the mannan antigen circulating in the serum of patients. Objective: To determine the clinical utility of immunosorbent assay detection of mannan antigen in human serum in pediatric patients. Material and methods: Analytical parameters of the ELISA test (Candida PlateliaTM AG®) were evaluated: sensitivity, specifi city, PPV, NPV, compared to the gold standard which is the blood culture. Results: 538 serum samples from 348 patients were enrolled with sepsis. A who underwent screening mannan Ag. 197 samples (36.6%) were positive for mannan Ag, for you, only 61 samples (11.3%) had positive mycological culture and only 19 (3.5%) blood culture positive for yeast. The sensitivity and specifi city of the test were 86.4% and 65.5% respectively, the PPV was 9.6% and NPV 99.1%. Conclusions: In the diagnosis of IC, the result of the test for mannan Ag should not be taken as a data point, it is essential to have clinical, mycological cultures and blood cultures. Because of the high negative predictive value of the test (99.1%) consider that this test would help rule out a probable CI. |
