| Abstract | Introduction: Polycystic kidney disease is, for the most part, an autosomal dominant genetic disorder with presence of renal cysts, which may result in loss of renal function and other manifestations such as hepatic cysts, cerebral aneurysm, and cardiovascular diseases. The horseshoe kidney is a more common renal anomaly caused by a defect in fusion during embryology. It is generally asymptomatic and may be associated with polycystic kidney disease, nephrolithiasis, renovascular hypertension, among others. The association between the two entities is rare, especially its diagnosis when, in most cases, it occurs in routine exams. Objective: To report the rare case of a horseshoe kidney patient associated with polycystic kidney disease. Materials and Methods: Review of medical records, photographic record of diagnostic methods and review of the literature. Results: Male patient, 76 years-old, hypertensive and with benign prostatic hyperplasia was admitted with oliguria, dysuria, abdominal pain for about a week with progressive worsening. He also reported fever and use of foley catheter for treatment of urinary retention for 9 months. After laboratory tests and abdominal tomography, he was diagnosed with acute pyelonephritis and renal and polycystic liver disease associated with a horseshoe kidney. Conclusion: We obtained with the reported case, and the literature reviewed a better characterization of the signs and symptoms of the horseshoe kidney in a patient with polycystic kidney disease.
