| Abstract | The fear of cancer récurrence (FCR) is a common reaction among those faced with the real and possible threat of a cancer returning and it has négative repercussion on the functioning and quality of life of cancer patients. Yet in spite of this observation, few systematic studies hâve been done on this issue partly perhaps because of the complexity and the heterogeneity of the phenomenon, but also because of the few efforts that hâve hitherto been made to conceptualize FCR and to develop measurement instruments that reflect its multidimensional nature. The présent thesis contains three empirical articles which had the following aims: (a) to develop and evaluate the psychometric properties of a new multidimensional questionnaire designed to assess FCR, the Fear of Cancer Récurrence Inventory (FCRI) and to evaluate its ability to detect clinical levels of FCR; (b) to explore the présence of spécifie profiles of FCR by adopting a multidimensional conceptualization of the phenomenon; (c) to describe the nature of intrusive thoughts associated with FCR and to investigate coping stratégies used to deal with it; (d) to document the psychiatrie comorbidity (current and past) and personality traits of patients presenting a clinical level of FCR. A provincial médical database was used to randomly sélect a pool of patients who had been treated for breast, prostrate, lung or colorectal cancer within the previous ten years. Selected patients (Study 1 : N = 600; Study 2 : N = 1984) received by mail a battery of self-report questionnaires. Some of thèse patients were invited to take part in a clinical interview (Study 3 : N = 60). Overall, the présent thesis confirais the importance of a multidimensional conceptualization of FCR. It proposes and validâtes a new questionnaire, the FCRI, that has been made available to evaluate the multidimensional aspects of FCR and to detect the présence of clinical levels (i.e. pathological) of FCR. It reveals the présence of four distinct profiles of FCR which vary according to its severity and the type of coping stratégies used. It describes the nature of intrusive thoughts associated with FCR. Finally, it explores the présence of psychiatrie comorbidity associated with FCR and proposes targets for psychological intervention. |
