Abstract	This study aimed to describe the profile of small cities health secretaries, the functions they perform and their knowledge and participation in the development of management tools. Crosssectional, descriptive, exploratory study conducted in 82 Small cities with a population of up to 20,000 inhabitants, of the areas covered by the 16th, 17eh, 18eh, 19th and 22nd regionals of healthcare of the north of the state of Paraná. We interviewed 72 Health Secretaries in their workplaces using a structured report, in the period of November 2013 to September 2014. We found a significant increase in the percentage of managers who had completed higher education; however, 40.3% did not have a graduation course. Most secretaries feel prepared to hold the office, and this preparation was acquired in daily work. Planning functions and management of the municipal health fund were highlighted since they are specific and essential functions. Knowledge of management tools and participation in their preparation demonstrated significant progress for the development of their role in management, but the low involvement with the specific tools of the municipal budget brings concern. There is a need for investment in regionalization, and that educational processes that are more appropriate to Unified Health System (SUS) are developed for management qualification through continuous education of municipal health secretaries.
