Tratamiento Endovascular Con Sistema Gdc De Aneurismas Del Segmento Carotídeo De La Arteria Comunicante Posterior: Experiencia En 100 Casos*

| Abstract | Objective. Some aneurysms arising from the posterior communicating segment of the internal carotid artery are difficult to treat with microsurgiry and are referred for their endovascular treatment. In a series of these cases we evaluate the immediate and delayed occlusion results, and the clinical follow up. Method. In a period of ten years, 98 patients with 100 aneurysms were treated with Guglielmi Detachable Coil system by the medical team of Centro Endovascular Neurologico Buenos Aires. Clinical and angiographic outcome and follow up were retrospectively reviewed. Results. Complete or subtotal occlusion was achieved in 53% and 27% of the cases while 19% had subtotal and partial occlusion. Of the 98 patients only 49 were clinically follow up (87.8% showed a good recovery, 5 patients showed moderate disability and 1 patient died of unrelated causes) and 53 of 100 aneurysms had follow up angiograms to evaluate stability of the occlusion observing that 26.4% had coil compaction. The degree of coil compaction considering the neck size in aneurysm rupture showed a 7.1% of coil compaction in aneurysms with small neck and a 50% in wide neck. No aneurysm bleeding was observed in the follow up. Conclusion. The results of this study indicate that endovascular treatment is a safe and effective therapeutic alternative in posterior communicating artery aneurysms, and although one fourth of the aneurysms showed coil compaction effective protection to bleeding or rebleeding was achieved. |
