| Abstract | The Family Health Team (FHT) Garden Candides, the city Divinópolis, Minas Gerais, through situation analysis identified a large number of cases of Diabetes Mellitus in the region that did not have adequate monitoring demonstrating the need for specific guidelines, such as food and proper adherence to treatment of diagnosed cases. Whereas diabetes is a serious medical event, the consequences of which, if left untreated, can lead the individual to death. Often the person with diabetes is unaware of their situation and unaware of the symptoms, making it necessary guidance for the specific treatment for this disease. This work aims to propose an action plan to ensure better care and follow-up of patients with diabetes mellitus, with a view to improving the quality of life of this group of users ascribed health strategy Candides family. To support the realization of the intervention project a literature review was conducted in the Virtual Library databases Health. Thus, the Candides Garden team proposed this intervention plan to improve accompaniment of decompensated diabetic patients of the unit's operating area Candides garden. |
