Abstract	espanolDenominamos enfermedad pulmonar obstructiva cronica a un conjunto de procesos clinicos que tienen en comun una obstruccion cronica y progresiva al flujo aereo, salpicada de episodios de reagudizacion (exacerbaciones o brotes). Estas exacerbaciones se hacen con el tiempo mas frecuentes e intensas deteriorando la funcion pulmonar. La principal causa de estas agudizaciones es la infeccion bacteriana. Existen multiples guias y documentos que abordan el manejo de esta patologia. Sin embargo, se centran fundamentalmente en el tratamiento durante la fase estable. Este documento realiza un abordaje del problema de la exacerbacion aguda con origen infeccioso desde una perspectiva multidisciplinar, centrandonos en el abordaje integral del proceso, y aborda la etiologia, resistencias a los antimicrobianos, estudios microbiologicos, la estratificacion del riesgo y el manejo terapeutico empirico inicial, antibiotico y concomitante. Ademas, incluye una aproximacion frente aspectos mas complejos como son el manejo de poblaciones especiales (ancianos, inmunodeprimidos) o del fracaso terapeutico ante el tratamiento instaurado. Por ultimo, se discuten especificamente temas mas controvertidos como la profilaxis de la infeccion o el tratamiento paliativo. Englishobstruction to airflow, with episodes of exacerbation. There are multiple guidelines and documents that statement the management of this pathology. However, they focus primarily on the treatment during the stable phase. This document addresses the problem of acute exacerbation due to an infection from a multidisciplinary perspective, focusing on the integral approach to the process, and including etiology, microbiological studies, resistance to antimicrobials, risk stratification and initial empirical therapeutic management (antibiotic and concomitant). In addition, it includes an approach to more complex aspects such as the management of special populations (elderly and immunosuppressed) or therapeutic failure. Finally, more controversial topics such as prophylaxis of infection or palliative treatment are specifically discussed.
