| Abstract | Patients with type 1 diabetes present an increased risk for cardiovascular diseases. The metabolic syndrome, a complex disorder defined by a cluster of interconnected factors including abdominal obesity, hypertension, dyslipidemia and insulin resistance, has been proposed to identify patients with type 1 diabetes at high cardiovascular risk. The metabolic syndrome has been identified in 8 to 45% of patients with type 1 diabetes depending on the definition and cohort studied. However, clinicians and researchers face several issues with the criteria for metabolic syndrome in patients with type 1 diabetes, questioning its value in routine care. For example, three criteria can lead to overestimation of the metabolic syndrome prevalence; the impaired fasting glucose criterion is irrelevant as it is automatically fulfilled, and the large use of antihypertensive and lipid-lowering medications for cardiac and renal preventive purpose can contribute to overestimate the prevalence of elevated blood pressure and elevated triglycerides. In cross-sectional studies, the metabolic syndrome has been associated mostly with increased risk for microvascular complications while the predictive value of the metabolic syndrome for micro and macrovascular outcomes was inconsistent in prospective cohorts. Identifying diabetic patients at increased risk for cardiovascular complications and early mortality is crucial from a preventive standpoint, however, for patients with type 1 diabetes, the current metabolic syndrome definition may not be the most suitable tool. The aims of the present article are to review the applicability and limitations of the metabolic syndrome in patients with type 1 diabetes and to discuss alternative avenues to identify high-risk patients. |
