| Abstract | PG. Kebon Agung is one of the industries located in Malang Regency. During 2013 to 2017 there were 33 working accidents caused by human error factors. This study aims to reduce human error. The result of the research show that the largest HEP value is 0.445 for the maintenance activity of the mill machine, 0.287 for cane cutter machine and 0.198 for the cane table machine maintenance activities. Based on the identification of the causes of errors using the highest HEP, it recommends the improvements, which are the improvement of the physical environment, the design tools, the making of procedures and the improvement of the operator's ability through the training and also improving the supervision. |
