| Abstract | SUMMARY In the 80s, studies demonstrated the beneficial effect of LTO (long-term oxygenotherapy) in survival, hemodynamic conse- quences and quality of life of patients with severe COPD (chronic obstructive pulmonary disease). The LTO in COPD is a life-long treatment, restrictive and costly. It is therefore important to respect the indications where blood gas criteria have been strictly defined and to determine the modality (especially oxygen source) that will allow to patients to have one hand optimal adherence and on the other hand a improved quality of life. Ambulatory oxygen ther- apy has greatly expanded in recent years due to technological advances and the generalization of respiratory rehabilitation programs. The diversity of devices placed on the market imposes a rigorous prescription, it can only be done after assess- ing the needs of the patient and evaluating the effectiveness of the equipment on the correction of hypoxia. Socio-economic, geographical and logistical considerations also come into the choice of the source of oxygen for which we must consider long-term maintenance: climate, patient's house (power supply, accessibility), organization of the supply of liquid oxygen and/or gaseous technical service. The solution is not unique, it must be adapted to the patient (severity of patient's condition, lifestyle) and the organization of health care system of the concerned country . |
