Abstract	Utility of Free/Total Serum Prostate-Specific Antigen Ratio in prostatic cancer studied with endorectal ultrasound biopsy Introduction: The tumor markers are important to decide to perform biopsies in the pr ostate cancer diagnosis. The goal of this article is to evaluate the diagnosis precision of FPSA (Free Prostatic Specific Antigen) as a tumor marker in patients with Prostatic Cancer suspicion and the FPSA ratio. Material and Method: A total of 248 patients aged between 38 and 88 years old wer e evaluated from November 2001 to June 2007. It was a prospective study analyzing the FPSA, the presence of echographic hypoecogenic lesion and the anatomopathologic final diagnosis to confirm either malig- nancy or benignity. Results: The selection of patients to perform Prostatic Endor ectal Biopsy (PEB) is a diary dilemma in Urology cab- inets. Since PEB is an invasive procedure it is necessary to adjust its indications. Until now the parameters that Urologists are using to indicate the biopsy are: Serum PSA, Clinics findings, and the Endorectal Ultrasound findings (EU). The ratio FPSA/Serum PSA must be considered with more emphasis. Conclusion: The identification of FPSA ratio alteration with a more confidential cut level adjusted to 0.14 can be more sensitive in the pr ostate cancer diagnosis and for the selection of patients to perform biopsies or avoid unnecessary invasive procedures.
