| Abstract | From the perspective of reproductive planning policy established in Brazil, fertility regulation assistance is fundamentally an act of health promotion, illness prevention, and promoting the exercise of reproductive rights (i.e. it does not deal with disease itself and, in this sense, refers mainly to primary care). Many issues and challenges involving reproductive planning assistance - whether they regard access, quality or integration with other health sectors - are not unique problems of this policy, but are part of a larger context, which is primary health care. Subject: Reproductive planning assistance in the Family Health Strategy for the City of Rio de Janeiro. Overall Objective: To consider the opportunities that the strategic model for the family could eventually provide for the improvement of assisted reproductive planning assistance. Specific Objectives: To describe the reproductive planning activities in one of the city's network of family health clinics and trace back the reproductive and contraceptive paths of those who make use of them. Materials and Methods: We conducted a qualitative case study in which we used two investigative procedures: A description of reproductive planning assistance in a Family Health Strategy clinic in the City of Rio de Janeiro, and tracing back the contraceptive trajectory of women who enrolled in the clinic's family planning program. We interviewed 48 enrollees who participated in an educational program. We conducted a descriptive evaluation of objective data taken from questionnaire responses and an analysis of contraceptive practices trajectories these participants' first sexual encounters. Results: The problems identified in reproductive planning assistance were the lack of attachment to the service, the lack of services that fulfill scheduled and unscheduled needs, the educational program model used, and the failure to conduct follow-ups with clients. All of these issues demonstrate that, using the traditional model, the same problems persist in FHS clinics and that these are related to a lack of integration, network coordination, and continuity of care, which undermine reproductive planning assistance. Conclusion: There is still a gap between the proposed FHS and the results it has really been able to produce. For primary care to be reorganized, it will be necessary to overcome these challenges. |
