| Abstract | Often, the conditions and causes of significant accidents in the chemical processing industry are repeats of prior events elsewhere within the industry, or sometimes even within our own company. The repeat incidents often occur because those involved did not have the knowledge of the learnings from the prior events. LyondellBasell has designed and implemented a High Learning Value Events (HLVE) incident learnings sharing process for these reasons. This session will demonstrate the importance and value of having both, a process (HLVE), and individuals, which search out, share, and institutionalize learnings from past incidents in order to prevent future recurrences. The presentation will review learnings from two case histories and provide an overview of LyondellBasell’s High Learning Value Events process. Individuals will be encouraged to become “Lessons Learned” leaders in their organizations, thereby contributing to the prevention of process safety accidents. |
