Abstract	espanolEste ano se cumplen 40 anos del inicio de la formacion MIR y puede ser un buen momento para reflexionar sobre las luces y sombras de nuestra formacion sanitaria especializada. Desde la publicacion del Real Decreto 639/2014 hasta su anulacion se llevaron a cabo numerosos esfuerzos encaminados a actualizar los programas formativos de las especialidades incorporando ademas medidas de evaluacion que permitian armonizar y sistematizar la supervision de la formacion. El momento actual nos plantea grandes retos, pero tambien nos brinda la oportunidad de seguir trabajando conjuntamente en los perfiles de los especialistas que nuestra sociedad demanda e impulsar la implementacion de un nuevo modelo formativo que contribuya a fortalecer nuestro sistema sanitario. EnglishForty years ago, the Spanish training system for physicians, commonly called MIR, was created and now is a good time to reflect on its strengths and weaknesses. In 2014, a legal document named Royal Decree (RD), regulated, among other things, the core curricula specialties (named ´´troncalidad´´). For the almost two years that the RD 639/2014 was in force, numerous efforts were made by many physicians to improve our Specialized Healthcare Training system through residency. Since its cancellation in 2016, many challenges have emerged that will require the commitment of all of us to implement a new training system that really contributes to strengthen our health system.
