Predictors And Patterns of Cognitive Decline Differ between Mild Cognitive Impairment in Parkinson’s Disease (PD-MCI) And Alzheimer’s Disease (AD-MCI)

| Abstract | Background: Neuropathological differences in the degenerative processes behind Alzheimer’s disease (AD) and Parkinson’s disease (PD) suggest that early cognitive impairment in the two diseases may manifest and progress differently. We examined patterns of cognitive deficits and decline over time, and compared risk factors predicting cognitive decline in each group. Methods: 127 participants (67 AD-MCI, 60 PD-MCI) underwent full neuropsychological assessments at baseline and at one year. The two groups were compared on baseline scores and longitudinal change in global cognition, episodic memory, attention, executive function, and language using the Mann-Whitney U test and multiple regression analysis. Logistic regression analysis was performed for AD-MCI and PD-MCI separately, to identify predictors of cognitive decline. Results: Based on comparable measures of global cognition, PD-MCI displayed poorer attention and executive function compared to AD-MCI. Both groups performed similarly on memory, and language. After one year, PD-MCI saw greater decline in executive functions than AD-MCI, but comparable changes in episodic memory. APOE4 possession and poorer baseline executive functions predicted cognitive decline in AD-MCI, but not PD-MCI. Conclusion: PD-MCI displayed poorer attention and executive function than ADMCI. Longitudinally, PD-MCI demonstrated greater decline in executive functions and comparable changes in episodic memory. Findings demonstrate that the variables often used by clinicians to predict cognitive decline in AD-MCI, such as APOE4, cannot be applied to PD-MCI. |
