Abstract	Nowadays, the changes in diet and lifestyle habits have been a major concern for the health, as we observe the increase in physical inactivity and poor nutrition, which can cause or aggravate diseases like Diabetes Mellitus (DM) . This research aimed to develop an action plan aimed at prevention, control and treatment of diabetes mellitus. In the construction of this work was held initially a bibliographical research to support the development of the plan. The DM is a disease that is not diagnosed and treated properly, entails a number of consequences such as blindness, heart disease, amputation, among others. In addition, treatment is complex and requires care and discipline. For this reason, the importance of educational work, seeking the control and treatment of patients with Diabetes Mellitus. With this research, it can be observed that the educational approach through projects and information for people with diabetes and the general population was important. Improving care also contributed greatly to the participants, as in meetings and activities developed, diabetics participated actively, and reports showed improvement in health status and care of them.
