The Prevalence of Antenatal Depression in South Asia: A Systematic Review and Meta-Analysis

| Description | Journal: SSRN Electronic Journal Background Antenatal depression remains largely unrecognised in South Asia. This study aimed to derive a synthesised estimate of the prevalence of antenatal depression in South Asia to inform policy, practice and future research. Methods We conducted a systematic review and meta-analysis. We searched 13 databases including PubMed, ScienceDirect, Scopus, Web of Science, PsycINFO, CINAHL, Global Health and other sources including grey literature for studies conducted between January 2007 and June 2018. Articles were screened in two stages against inclusion criteria with titles and abstracts screened first followed by full-text screening. Methodological quality was assessed using the 'Guidelines for evaluating prevalence studies'. We estimated pooled prevalence using random-effects model and conducted subgroup analyses. Funnel plot and Egger test were used to assess publication bias. (PROSPERO registration CRD42017078795) Findings Thirty-three studies involving 13,087 pregnant women were included. Overall pooled prevalence of antenatal depression was 24·57% (95% CI; 19·34-30·69). Studies showed a high degree of heterogeneity (I2=97·55%), with evidence of publication bias (p=0·722). After excluding studies with moderate quality and higher risk of sampling bias, the pooled prevalence was 20·12% (95% CI; 15·36-25·91) and 15·87% (95% CI; 8·57-27·5) respectively. The prevalence rates for India and Sri Lanka were lower compared with overall prevalence whereas rates for Pakistan and Nepal were higher. Interpretation Robust prevalence studies are sparse; but available data suggests one in four pregnant women to experience antenatal depression in South Asia. Findings highlight the need for policy and practice to recognise antenatal depression as a threat to maternal mental health. Funding: There was no funding source for this study.
