| Description | Five cases of patients presenting binocular vision problems after refractive surgery have been analysed. In one patient, a latent superior oblique palsy decompensated after LASIK of the most myopic eye to achieve monovision. A second patient, operated for a fourth nerve palsy ten years ago, presented a recurrence of his palsy after bilateral LASIK for myopia. The third patient complained of intermittent diplopia caused by aggravation of a pre-existing intermittent esotropia that worsened after LASIK for hypermetropia. The fourth patient had permanent diplopia after LASIK of his highly anisometropic and exotropic eye. Finally, the fifth patient experienced a decrease in fusion and stereopsis at the time he became anisometropic after bilateral PRK and LASIK. These cases clearly illustrated the need of an accurate preoperative orthoptic examination in order to prevent postoperative complaints secondary to changes in binocular vision.
