| Description | Obstetric anal sphincter injuries (OASIS) are defined as perineal trauma involving the anal sphincter complex during childbirth. The reported incidence of OASIS varies between 1.7% and 18% of vaginal births. Over the past decade, there has been an increase in the reported cases of OASIS worldwide. Allegations of negligence include erroneous diagnosis of the true extent and grade of the injury, failure to perform a rectal examination prior to suturing a perineal tear, overlooking consideration to carry out a caesarean delivery or to perform or extend the episiotomy, inability to do the repair and inadequacy of the repair. Occipitoposterior position is associated with incomplete flexion of fetal head, an increase of the presenting diameter and a prolonged second stage of labour. Precipitate labour is associated with cervical, perineal, labial and urethral injury, due to inadequate time for the maternal tissues to adjust to delivery forces and the failure to control delivery with an episiotomy, when necessary.
