| Description | Allergic rhinitis is a common IgE-mediated inflammatory disease of the nose and paranasal sinuses. Rhinitis is often undertreated and this negatively impacts the quality of life. Rhinitis is classified as intermittent, persistent, mild, and moderate-severe. Clinical manifestations include nasal congestion, rhinorrhea, postnasal drip, nasal pruritus, sneezing, and itchy/watery eyes. A thorough history is important focusing on symptom onset, severity, environmental exposures, treatments used, and response to treatments. Physical examination should focus on signs of atopy including allergic shiners, Dennie-Morgan lines, allergic salute, allergic facies, nasal mucosal changes, nasal polyps, wheezing, and signs of atopic dermatitis. Differential diagnosis of rhinitis includes nasal anatomical abnormalities including deviated nasal septum, adenoid hypertrophy, foreign bodies, nasal tumors, cleft palate, and cerebrospinal fluid leak. Investigations include skin prick testing to relevant aeroallergens and specific IgE immunoassays. Management focuses on allergen avoidance measures, pharmacotherapy, and allergen immunotherapy. Pharmacotherapy includes oral and intranasal H1 antihistamines, intranasal glucocorticoids, antileukotrienes, intranasal antimuscarinics, and intranasal mast cell stabilizers.
