INTRODUCTION The Long-tenn Intervention with Pravastatin in Ischaemic Disease (LIPID) study was designed at a time when cholesterol-lowering therapy was controversial, even for secondary prevention of coronary events in patients with preexisting coronary heart disease (CHD). Most of the early intervention studies had been conducted in patients with modest hypercholesterolemia with interventions tested for short periods. Although these trials collectively demonstrated that an average cholesterol reduction of about 0.6 mmol/L (23 mg/dL) resulted in approximately 10% reduction in CHD deaths, an insignificant increase in non-CHD deaths due to cancer and trauma was also observed (1-3). Considerable doubt was cast on the overall benefits of cholesterol lowering with regard to all-cause mortality in patients with high cholesterol levels (4). There was also a paucity of data concerning the effects of more substantial cholesterol lowering, particularly in patients with relatively nonnal cholesterol levels.
