| Description | Neglected dislocation of the hip joint in young adults is difficult because of pain and leg length inequality. Numerous salvage procedures include total joint replacement and pelvic support osteotomy. A percutaneous subtrochanteric valgus femoral osteotomy, stabilized with an Ilizarov external fixator, and then a more distal femoral osteotomy for correction of valgus and for lengthening can be stabilized with a standard Ilizarov fixator, but it is easier to use a Taylor spatial frame, which will be the method described stepwise in detail in this chapter.
