| Description | The rapid growth in the number of clinical proton beam facilities allows an increasing number of patients to benefit from this unique treatment modality. Although the dosimetric benefits of protons are well understood, when compared to photons, and can be optimized during treatment planning [1–3], the radiobiological effects and underlying mechanisms and systemic response in vitro and in vivo are more uncertain [4,5]. It is recognized that the variable relative biological effectiveness (RBE) of protons holds the potential for significant improvement in the therapeutic ratio, and therefore treatment outcomes, in terms of both reduction/avoidance of normal tissue complications and increase in tumour control. As tools for biological optimization are being developed [6,7] and availability becomes more widespread, a better understanding of the limitations of current models [5,8,9] and their validity in clinical practice is required. Our current knowledge of RBE effects is primarily based on in vitro cell culture experiments [9,10]. Exploring the biological effects through experimental observation and verification of radiobiological models in vivo requires dedicated preclinical proton beam facilities [11,12], of which few currently exist. Monte Carlo (MC) simulation plays an important role in modelling these research facilities. It is also increasingly important to fully understand the differences between clinical proton beams and lower-energy proton beams utilized for preclinical small animal research.
