| Description | There are approximately 320000 disabled children in England, the majority of whom live at home with their parents. The prevalence of severe disability and complex needs in children has risen. The great majority of disabled children live with their families and it is the families who assume day-to-day responsibility for caring for their child. The chapter addresses a number of issues in relation to caring for disabled children and their families in primary care. These are: promoting social inclusion, early years provision of services, providing direct support to families, transition into adulthood, commissioning services, and training and development of staff. In order to meet the needs of disabled children, it is particularly important that PCTs work with other agencies to ensure that the workforce has the necessary skills and competencies. Training should include disability awareness, equality and inclusion issues. They should ensure effective collaborative practice across primary care, including encouraging continuing professional development on a multidisciplinary basis.
