| Description | Perthes' disease is a self-limiting form of osteochondrosis of the capital femoral epiphysis of unknown aetiology that develops in children commonly between the ages of five and 12 years. Children under five and adolescents are affected much less frequently. The blood supply to part or all of the epiphysis becomes interrupted, leading to necrosis of the involved part. A varus derotation osteotomy is one of the most widely used methods of achieving containment. It entails an intertrochanteric or subtrochanteric osteotomy wherein the distal fragment is adducted and externally rotated to the same degree of abduction and internal rotation of the hip required to achieve containment. If passive internal rotation of the hip remains restricted after a period of traction or after the broomstick cast is removed a varus extension osteotomy is performed. A Chiari osteotomy is another way of improving coverage of the femoral head. Book Name: Paediatric Orthopaedics |
