| Description | Assessments are an essential prerequisite to caring for frail and ill older people. In the UK, formal assessment of continuing care need is required for: (i) placement in Care Homes; (ii) review at the time of transfer; and (iii) review at stipulated intervals thereafter on an ongoing basis. Health and social care assessments result thus facilitate the placement of an older person in a suitable 'fit-for-purpose' facility.
