| Description | An advance directive is a statement of treatment preferences as an indication of a patient's wishes should the capacity for decision-making be lost in the future. Based on the principle of autonomy, it aims to project this forward into possible future mental incompetence. Advance directives were first introduced in the United States of America following the case of Karen Quinlan. Karen was aged 21 when in 1975 she developed a persistent vegetative state after taking tranquillisers and alcohol. She was maintained on a respirator and by naso-gastric feeding. A source of concern on advance directives is the Law Commission report on Mental Incapacity. Some advance directives however are less ambitious and confine themselves to a values inventory indicating the subject's likely views if they were to be subject to certain circumstances. Although there is as yet no legislation recognising advance directives, it is thought that common law precedent may well give them legal recognition. Book Name: Medical Ethics and the Elderly |
