| Description | This chapter provides an overview of population health in Australia and assist with understanding population health approaches to interventions. It emphasises the rationale for working at the community/population level, and explains the difference between individual-level and population-level approaches to interventions. The social-structural level influences the psychosocial factors, which in turn influence the behavioural pathways that lead to cardiovascular disease and type 2 diabetes.
