| Description | The transthyretin (TTR) V122I allele is associated with cardiac amyloidosis. Nearly all affected patients have had Black African ancestry. The TTR V122I allele is carried by 3.9% of African-Americans, with equal prevalence throughout the U.S.A. (1). Most clinical information about allele carriers comes from individual case reports, and little is known about carriers' disease penetrance or early disease manifestations. It is also known that cardiac amyloidosis is more prevalent in African-Americans than in Caucasian Americans (fig. 1) (2). In a previous study, we have shown that the excess prevalence of cardiac amyloidosis in Africans is cardiac ATTR, largely V122I ATTR (3). While these studies have established the association of the TTR V122 gene with clinical disease, the allele's penetrance in causing clinical disease, and the early findings in allele carriers on noninvasive testing, are unknown.
